long fue hair transplant

Long FUE is a hair transplant in which the grafts are extracted and implanted without cutting the hair short. In a standard FUE operation the donor hair is trimmed to about 1 mm. In Long FUE each follicle keeps its full length, so the doctor can see the direction, curl and fall of every hair while placing it, and you leave the clinic with a hairline that already shows its final shape. It suits patients who want to hide the operation from day one and need a hairline that blends exactly with their existing hair. In Turkey, Long FUE typically costs $2,400 to $5,000, compared with £10,000 to £15,000 in the United Kingdom and $16,000 to $30,000 in the United States for a session of about 2,000 grafts.

I am Dr. Mehmet Erdoğan, co-founder of Smile Hair Clinic in Istanbul and an associate member of the International Society of Hair Restoration Surgery (ISHRS). Long FUE is one of the most demanding techniques in hair restoration, because every graft is handled with a long hair shaft attached. When it is used for the right patient, it gives a level of design control and discretion that a shaved operation cannot match. This page explains how Long FUE works, how it differs from an unshaven transplant, who it suits and what it costs.

What Is Long FUE?

Long FUE, also called long hair FUE, is a variation of follicular unit extraction in which the hair on each graft is not cut before extraction. The doctor removes the follicles one by one from the back of the head, exactly as in standard FUE, but each graft comes out with its hair shaft still several centimetres long. The same long grafts are then implanted into the thinning area.

Because the hair is still attached, the doctor works with what the result will actually look like, not with tiny stubs. The angle, the natural curl and the way each hair falls are visible during placement, so the new hairline can be matched to the hair around it with great precision. Smile Hair Clinic explains the idea in the short video below.

How Is Long FUE Different From Unshaven FUE?

The two are often confused, but they solve different problems. An unshaven hair transplant is about not shaving your head. The donor area is trimmed only in hidden strips or tiny windows, and the existing hair covers the work. The grafts themselves are still short. Long FUE is about the grafts keeping their length, which changes what the doctor can see during the operation and what you see in the mirror straight afterwards.

Long FUE Unshaven FUE Standard shaved FUE
Donor hair before extraction Left long on the extracted grafts Trimmed in hidden strips or windows Shaved to about 1 mm
Grafts at implantation Long hair shafts attached Short Short
Preview of the result Immediate, the hairline is visible on day one None until regrowth None until regrowth
How visible the operation is Very discreet Discreet Clearly visible for several weeks
Typical session size Small to moderate Small to moderate Small to very large
Operation time and cost Highest Higher than shaved Standard

In practice the two techniques are often combined. A patient may have an unshaven donor area and long grafts at the hairline, which gives both a hidden donor and an immediate preview at the front.

What Happens to the Long Hairs After the Operation?

This is the question almost every patient asks, and the honest answer matters. The long hairs you see on day one are not the final result. As in every hair transplant, most of the transplanted hair shafts fall out within the first weeks while the follicle stays safely in the skin. New hair then grows from the same follicle over the following months.

So the value of Long FUE lies in two things. During the operation, the long hair lets the doctor place each graft at the correct angle and direction. In the first weeks, it hides the recipient area and gives you a realistic preview of the shape and density of the new hairline, so you can return to work and social life with far less attention.

Who Is a Good Candidate for Long FUE?

Long FUE suits patients who need a precise, natural hairline and cannot afford to look operated. Typical candidates include people in public-facing jobs, actors, presenters and business owners, patients who want to keep their current hairstyle, and those with early to moderate hair loss, around Norwood Type 2 to Norwood Type 4, where the work is concentrated at the front. It is also useful for refining a hairline or adding density between existing hairs, and for female hair transplant patients, who usually do not want any part of the head shaved.

A strong donor area with good density is essential, because extracting long grafts requires a clear view between the hairs. Long FUE is less suitable for advanced hair loss that needs a very large number of grafts in one session, for severe donor thinning, for some scalp conditions and for very tightly curled hair, where long curved follicles are difficult to extract and place intact. Patients with tightly curled hair can read more on the Afro hair transplant page. For large areas, a shaved sapphire FUE operation is usually the better route.

How Is a Long FUE Hair Transplant Performed?

A Long FUE operation is performed under local anesthesia and usually takes about seven to nine hours, longer than a shaved operation of the same size. Each step needs more care, because the long hair has to be kept in order and protected throughout.

Planning and hairline design come first. With your hair at its natural length, we study the direction, curl and parting of your existing hair and draw the new hairline to match it. The donor area is then numbed and the grafts are extracted one by one with their hair intact. Extraction is often done with slow, careful technique, sometimes manual, so that the follicle and the long shaft come out undamaged. You can read more about this approach on the manual FUE page.

The grafts are sorted and kept in a cooled storage solution. Implantation is frequently done with Choi implanter pens, which let the doctor place long grafts at a precise angle and depth without opening channels first. This is the same instrument used in a DHI hair transplant. As each graft goes in, the long hair shows immediately whether its direction matches the surrounding hair, and small corrections can be made during the operation itself.

What Does Recovery Look Like After Long FUE?

Recovery follows the same biology as any FUE transplant, but it is far less visible. The long transplanted hairs and your existing hair cover most of the small crusts, and the donor area is hidden under the hair that was left in place.

Time after surgery What you will notice
Days 1 to 3 Mild swelling and small crusts, mostly hidden by the long hair
Days 4 to 14 Crusts soften and fall off with gentle washing
Weeks 2 to 8 Most transplanted hair shafts shed while the follicles stay in place
Months 3 to 6 Fine new hairs start to grow and gradually thicken
Months 9 to 12 The result matures, sometimes continuing up to 18 months

Most patients return to office work within a few days. Sleeping position, washing and protecting the long grafts need a little extra care in the first week, and all the day-to-day instructions are collected in the patient guide.

How Much Does Long FUE Cost?

Long FUE is the most labour-intensive form of FUE, so it is priced above a standard shaved operation almost everywhere. In Turkey, a Long FUE hair transplant typically costs between $2,400 and $5,000, depending on the clinic, the number of grafts and who performs the operation.

In the United Kingdom, clinics usually charge 30 to 50 percent more for unshaven and long hair techniques than for shaved FUE, and a fully unshaven or long hair procedure typically costs £10,000 to £15,000. In the United States, long hair FUE is commonly priced at $8 to $15 per graft, so a session of about 2,000 grafts often reaches $16,000 to $30,000.

Country Typical Long FUE price
Turkey $2,400 to $5,000
United Kingdom £10,000 to £15,000
United States $16,000 to $30,000 for about 2,000 grafts

The package options and who performs each one are explained on the hair transplant cost in Turkey page, and the whole procedure step by step on the hair transplant in Turkey page.

What Are the Risks of Long FUE?

Long FUE is safe when it is performed by an experienced team in a licensed, sterile facility. The usual effects are the same as in any FUE operation, with temporary swelling, crusts, itching and numbness that settle within days or weeks.

The specific risks come from the difficulty of the technique. Handling long grafts takes more time outside the body, and poor handling can damage follicles and lower graft survival. Trying to fit too many long grafts into one session can also lead to fatigue and lower quality. This is why the session size is planned carefully and why the technique should only be done by a team that uses it regularly. Before booking, ask how often your doctor performs Long FUE and to see results similar to yours. You can read about my background on the about me page.

Frequently Asked Questions About Long FUE

Do I need to shave my head for Long FUE?

No. The grafts keep their length and the donor area is usually left unshaven, so most people around you will not notice the operation.

Will the long hairs stay after the operation?

Most will shed within the first weeks, which is normal. The follicles stay in place and grow new hair from about the third month.

Is Long FUE the same as unshaven FUE?

No. Unshaven FUE avoids shaving the head, while Long FUE keeps the hair on the grafts themselves, which allows an immediate preview of the hairline. The two can be combined.

How many grafts can be transplanted with Long FUE?

Sessions are usually smaller than shaved operations, often up to about 2,000 to 2,500 grafts, depending on your donor area and the area to be covered.

Is Long FUE more painful than standard FUE?

No. After the local anesthetic takes effect, the experience is the same. The operation simply takes longer.

Why does Long FUE cost more?

Every graft takes more time and care to extract, store and implant with its hair attached, so the operation is longer and needs a more experienced team.

Can women have Long FUE?

Yes. Many women prefer it, because it lets them keep their hairstyle and hide the operation.

How long should I stay in Istanbul?

Most patients stay about three days, which covers the consultation, the operation and the first wash.
